Your first visit
Before you come in
No referral needed to start: Hawaiʻi law lets you see a physical therapist directly. Some insurance plans still want a doctor’s order to pay for it, so once you request a time, we’ll help you check what’s needed before you arrive. See Insurance & pricing for the details on your specific plan.
After you book, we’ll text you a secure link to fill out your intake forms online. Paper forms are also available at the front desk.
What to bring
- Photo ID
- Your insurance card, if you’re using insurance
- A credit or debit card, for our card-on-file policy
- A list of your current medications
- Any imaging reports (X-ray, MRI) related to why you’re coming in, if you have them
- Your referral or prescription, if your plan requires one
Appointment policies
- Call to cancel or reschedule at least 24 hours ahead to avoid a $50 cancellation fee.
- As of 2026, we keep a credit or debit card on file for the no-show and same-day cancellation fee.
- Arriving more than 15 minutes late may mean rescheduling or waiting for the next opening.
- Estimated copays are due on the day of service.
What to wear
Wear something you can move in. If we’re working on your shoulder or knee, loose or athletic clothing that gives us access to that area makes the evaluation easier — think what you’d wear to the gym or the beach.
Your evaluation
Your first visit is a full, hands-on evaluation. We’re not just looking at where it hurts — we’re finding what’s actually driving the problem, whether that’s your shoulder, your hips, your posture at work, or how you’re moving in the water. Expect manual assessment, some movement testing, and real conversation about what you’re trying to get back to.

Your plan
You leave your first visit with a plan, not just an appointment card. That means hands-on treatment that day plus a home exercise program you can start right away. We’ll talk through what your plan realistically looks like before you commit to anything.
